Insurance – I am a provider
on the Blue Cross/Blue Shield (BCBS), Private Healthcare
Systems (PHCS), and Seton Health Plan (SHP) insurance
networks (only the PPO plans, not the HMO plans).
You will need to contact me to see if I am accepting
new insurance clients.
If I am not a provider on your insurance plan but
you would like to seek insurance payments for services,
your insurance company will likely consider me an
"out of network" provider and reimburse
you for a portion of the fee. I can provide you with
an invoice for the services that you receive that
you can submit to your provider for reimbursement.
The amount that the insurance company reimburses you
will depend upon the particular insurance company
and plan.
Therapy & Consultation
– My fees for therapy and consultation are competitive
with those of most psychologists. Please contact me
for specific fee quote.
Evaluations – Evaluations
include an initial interview, a review of background
information and history, standardized testing in various
areas depending upon the particular needs of the client,
a report consisting of a review of history, test findings,
diagnoses, and recommendations, and a one hour feedback
session. My fees vary depending on the nature and
scope of the evaluation. Please contact me for a specific
fee quote.
Methods of Payment - I accept credit
card payments through Visa and MasterCard as well
as personal checks.
